| Child to Carry Own Medicine | A healthcare professional has advised that your child should carry their own medicine in school and they are able to administer the medicine independently. |
| Emergency Salbutamol Inhaler | Your child has asthma and you consent for the school to administer an emergency inhaler if your child's is unavailable. |
| Exceptional Circumstances Request |
You are requesting to take your child out of school during term time. (Note: you do not need to complete this form where the day is exclusively set apart for religious observance by the religious body to which you belong, e.g Eid. See our Interfaith page). |
| Permission to Walk Home | Your child is in Year 5 or Year 6 and you would like them to walk home. |
| Prescribed Medicine | You are requesting the school to administer prescribed (not over the counter) medicine to your child. |
| Pupil Leaver | Your child is leaving Sacred Heart. |
| Special Medical Diet Request | Your child is receiving school dinners and has a food allergy. |
