Your child has asthma and you consent for the school to administer an emergency inhaler if your child's is unavailable. |
| Exceptional Circumstances Request |
You are requesting to take your child out of school during term time. |
| Permission to Walk Home |
Your child is in Year 5 or Year 6 and you would like them to walk home. |
| Prescribed Medicine |
You are requesting the school to administer prescribed (not over the counter) medicine to your child. |
| Pupil Leaver |
Your child is leaving Sacred Heart. |
| Special Medical Diet Request |
Your child is receiving school dinners and has a food allergy. |
