Emergency Salbutamol Inhaler

Complete this form if your child has asthma and you consent for the school to administer an emergency inhaler if your child's is unavailable. You will have already informed school that your child uses an asthma inhaler. We follow advice from the Department of Education and Asthma UK to support children with asthma.

My child has a working, in-date inhaler, clearly labelled with their name, which they will bring with them to school every day

I have provided a spare (working, in-date inhaler, clearly labelled with their name) which will be held in school

In the event of my child displaying symptoms of asthma, and if their inhaler is not available or is unusable, I consent for my child to receive salbutamol from an emergency inhaler held by the school for such emergencies
