Forms for children already in school
| Form | Complete this form if... |
| Child to Carry Own Medicine | A healthcare professional has advised that your child should carry their own medicine in school and they are able to administer the medicine independently. |
| Complaint | You have read the Complaints Policy and Procedure on our Policies page, and wish to direct a complaint about the school to the Headteacher. |
| Emergency Salbutamol Inhaler | Your child has asthma and you consent for the school to administer an emergency inhaler if your child's is unavailable. |
| Exceptional Circumstances Request | You are requesting to take your child out of school during term time. |
| Free School Meals Application | You are on benefits or a low income and would like your child to receive free school meals. You do not need to complete this form if you are already receiving help with your Council Tax or Housing Benefit from Leeds City Council. |
| Permission to Walk Home | Your child is in Year 5 or Year 6 and you would like them to walk home. |
| Prescribed Medicine | You are requesting the school to administer prescribed (not over the counter) medicine to your child. |
| Pupil Leaver | Your child is leaving Sacred Heart. |
| Special Medical Diet Request | Your child is receiving school dinners and has a food allergy. |
